I would like help writing a letter to my employer Please

I am minibus driver I have been working in this company for 2 years,the last few months I had back problem,I went to see my GP and he refered me to the hospital for physiotherapy and a letter to my employer explaining the problem and gave me drowsy tablets to help with the pain and asked me not to drive when taking them ,I aproached my manager asking him to work as a partimer while I am attending the physio center he refused. I would like to write him a formal letter and I need help with that please..--Answer1:Which country do you live in? It is highly likely that you have grounds for contesting this, but I couldnt help unless its an area where I know the law..--Answer2:You should be able to go on the sick for 6 months on full pay if you have full sick entitlement so you need to know your full working rights, i dont think you can be paid off or made redundant through sickness so in your letter you need to outline that you know your legal rights, but as its a driving job with a psv license/lgv or appropriate license you need to know your rights, even a diabetic can have their license taken from them by the DVLA so in your letter you need to be 100 percent sure your have legal backing but your boss is legally bound by law to report any medication you take to the DVLA and to his insurance company.
